Advanced Disposal offers a comprehensive health and welfare plan to all of its full-time (30+ hours/week) employees. This plan includes major medical, dental, vision, life, supplemental life, spouse life, child life, an Employee Assistance Program (EAP), voluntary worksite benefits and a qualified retirement plan (401k). Advanced Disposal offers a Blue Cross and Blue Shield of Florida PPO plan as our major medical plan. The partnership with BCBSFL offers a large nationwide network of providers. Our plan offers $600 / $1,800 calendar year deductible and 85% / 15% co-insurance. Our plan is ACA compliant.
We are excited to offer the Teladoc program! Teladoc allows you to speak with a physician over the phone or via online video to help with minor illnesses such as cold and flu symptoms, allergies, pink eye, bronchitis or respiratory illness and much more! No need to take time away from work or travel to the doctor. This convenient, NO copay program is available 24-hours-a-day, 7-days-a-week and 365-days-a-year.
Advanced Disposal offers our employees comprehensive PPO dental coverage through Delta Dental. The Delta Dental network is a large network of discount and in-network providers. Our dental benefit includes $75 / $150 deductible and pays a 100% benefit (deductible waived) for Preventive Services, 80% for Basic Services and 50% benefit for Major Services. Through Delta, we offer a child dependent orthodontic benefit up to $1,500.
Advanced Disposal’s vision insurance provider is EyeMed. We offer a Basic Discount benefit for vision services for all employees and offer a Buy-Up option in which employees can contribute a small premium amount for more comprehensive coverage. The benefit includes: routine exam copay at $15, contact lenses up to $130 for in-network services and frames benefit for single, bi-focal and tri-focal lenses paid in full after copay. Frame allowance is at $130 but continues to offer a 20% benefit if over allowance.
Advanced Disposal provides a term life insurance policy through Sun Life, at no cost to full-time employees, at a benefit of two (2) times their base annual salary. In addition, employees may purchase up to five (5) times their annual base salary up to $1,500,000 in coverage of supplemental insurance on themselves, and up to $100,000 (not to exceed employees coverage) of their covered amount on their spouse. Child Life is offered at a flat amount ($2.30 per month) on a $10,000 policy and $5.75 on $25,000 of coverage. The $2.30/$5.75 rate is the same rate, regardless of the number of children covered.
LONG TERM DISABILITY
Long Term Disability coverage is provided at no cost to employees to provide a 40% of base salary disability coverage to our full-time employees. Employees may purchase an additional 20% benefit as well.
FLEXIBLE SPENDING ACCOUNT
We offer health and dependent care accounts with convenience of a debit card through our medical plan provider, Blue Cross Blue Shield of Florida. Employees can elect up to $2,500 for their qualified health expenses and up to $5,000 for dependent care expenses.
At Advanced Disposal, we want all of our employees to have the opportunity to retire with confidence. We have chosen VOYA Financial as our 401(k) administrator, one of the premier administrators in the retirement industry. We offer a competitive company match and vesting schedule. When eligible employees contribute up to 6% of their salary, those funds will be matched up to 3% with company matching funds. Employees are eligible to participate in our 401(k) 90 days after their date of hire and are fully vested at 5 years of service.
EMPLOYEE ASSISTANCE PROGRAM (EAP)
Advanced Disposal also cares about our employees’ well-being. We offer the EAP program through Lincoln Financial for all employees and their qualified dependents. This free, confidential benefit provides:

VOLUNTARY WORKSITE BENEFITS
Advanced Disposal has partnered with Allstate Benefits to offer the following supplemental insurance policies: Group Accident, Group Hospital Indemnity and Group Critical Illness with Cancer coverage. Employees that enroll in any of these plans can receive lump sum cash benefits for qualified health care events to help defray the costs associated with accidents, health events and wellness visits.
Advanced Disposal encourages our full-time employees to periodically take time off for rest and relaxation and provides to it’s full-time employees a paid vacation benefit based on length of service with the Company. The following schedule applies:
After one (1) year – five (5) vacation days
After two (2) years – ten (10) vacation days
After five (5) years – fifteen (15) vacation days
After twelve (12) years – twenty (20) vacation days
In addition to paid vacation days, Full time employees will also receive up to four (4) paid personal days per year
